• COMTREA Comprehensive Health Center - Behavioral Health - Primary Care - Dental Care

     Students can receive dental care, primary care, mental health, psychiarty and counseling services at COMTREA Health office located on the  Valley Middle School  campus through collaboration between COMTREA and the Northwest School District. 

    Our School-based health center promotes healthy living by: 

    • Being accessible and affordable to students and their families 
    • Increasing health education 
    • Providing convenience for parents and care-givers 

    Services are available to students and their families Monday-Friday from 7:30am-6:00pm, and available to the community Monday-Friday from 2:30pm-6:00pm. COMTREA accepts most insurance plans, Medicaid, Medicare and self-pay. An income -based sliding fee scale is offered to reduce costs for eligible applicants.  Additionally, COMTREA offers assistance  in navigating Medicaid applications. 

    Student transportation between District Schools and COMTREA Health Center located on the Valley Middle School Campus  is available during school hours by a district staffed liaison.  Written consent is required for services and transportation.  See forms below.

    In addition to the school-based health center, the COMTREA Tooth Titans mobile program periodically visits campuses to provide on-site dental services which increases accessibility and serves more students.

    Tooth Titans mobile program offers two options for families; 

    1. Dental screening including fluoride treatment/sealants and dental education
    2. Dental Exam includes above plus, comprehensive exam by dentist and x-rays, teeth cleaning - Insurance billed.

     COMTREA is a Federally Qualified Health Center and a Certified Community Behavioral Health Organization with 20 locations across Jefferson County. If you don't have insurance, sliding fee scale is available contact the Comtrea Central scheduling  for assistance. 877-266-8732
